Referral Services is the 'next level of care' after first experiencing the Grace Care Center's Choice Food Pantry or attending an English Class for a minimum of 30 days.

Meet one-on-one with a volunteer 'Advocate' to share your personal story, and receive guidance on attainable steps for incremental improvement leading towards a more healthy lifestyle.

How Do I Get Started?

To get started with Referral Services, please ask for a paper application from a Care Center staff member during normal operating hours.

After returning your completed paper application, you'll sit down with a volunteer (or staff member) for a brief chat to discuss your specific needs.

Referral Services is a branch of the Grace Care Center where friends meet one-on-one with a volunteer 'Advocate' to share their story, and receive personalized coaching on how to move towards a more sustainable, healthy lifestyle.

Our team of volunteer Advocates is ready will sit with you, listen to your story, and help identify one or two steps to begin taking towards sustainable life change. Oftentimes, our volunteers are able to refer you to someone in the community for additional guidance.

Whether you're seeking legal advice, need budgeting guidance, or seeking to improve your emotional and social health, we'd be honored to walk alongside you in your journey!


Use of Referral Services is limited to friends who have been utilizing the Grace Care Center Choice Food Pantry -or- attending ESL Classes for a minimum of 30 days.

The Co-op Program is for friends who seek more than just a one-time referral to an outside service provider.

Our team of dedicated, volunteer 'Advocates' will meet with you on a monthly basis to walk alongside in helping to suggest and implement steps that lead towards sustainable life change.

You'll have a one or two volunteer 'Advocates' that will listen to your story, suggest ideas for healthy habits, and walk with you as you experience incremental improvements towards a holistic, healthy lifestyle.

To be considered for the Co-op Program, you must request a paper application from a Care Center staff member during normal operating hours. Once you submit your application, you'll sit down with a volunteer to access your needs before being admitted to the program.

You can expect to sit with a volunteer 'Advocate' who will listen to your personal story, and work hard to offer attainable ideas and steps that will lead to incremental improvement, and eventually, a healthy and sustainable lifestyle.

Although additional meetings may be suggested by your volunteer 'Advocate,' you can expect a minimum of one in-person meeting each month.
