Welcome to the City Relief May 2023 (Young Adults) Trip Application!

Here, you'll be able to submit your initial application for approval to participate in this trip.

The Process for Trip Acceptance

Your acceptance to participate in this trip is a two part process:
  1. Part 1 is this Application form. After you complete this Application form, it will be reviewed by both the Trip Leader and the Grace Church staff
  2. Part 2 is a Registration form & down payment. If your Application is approved, you'll next be asked to fill out a detailed Trip Registration form (In addition to details, this Registration form will collect a $100 down payment)

A Few Things to Understand...

  • Two personal recommendations are required for trip approval. You will be prompted to enter the name and email of two personal references when completing this application.
  • All trip applicants ages 18 and over will be subject to a background check (if not already on file.)
  • Upon trip leader approval:
    • All trip participants must sign a waiver of liability to take part in the trip. Participants under the age of 18 will need all legal guardians’ signatures on the liability waiver.
  • The trip cost includes all airfare, lodging, food in-country, transportation in-country, emergency international medical insuranc,e and any other incidentals purchased by the ministry partner for the team (i.e. water, supplies, etc.).
  • The trip cost does not include any required or recommended immunizations or medications for travel, passport application/renewal fees, or souvenirs.

Application Instructions

  • Please allow 20-30 minutes to complete this application.
  • This application will ask for your basic contact info, two personal references, and your personal reflections on a number of trip-specific questions.

I have read these instructions. I understand that anyone 18 and over will need to complete a background check if a current one is not on file at Grace Church and will be required to sign a liability waiver. I also understand anyone under the age of 18 will need all legal guardians’ signatures on the liability waiver.
